- Joining our membership sets you up for success by providing the opportunity to check in whenever you need to in order to optimize your migraine care and answer, in detail, any questions that might arise
- Medications and testing are only ordered for members. Calls and messages from non-members are welcomed and are billed at Dr. Myers' hourly rate.

- Direct access to Dr. Myers via messaging, telephone calls (up to 10 minutes, any issue requiring a call longer will require a follow-up visit to be scheduled).
- Up to two forms filled out per quarter for the patient (extra paperwork or forms done at a discounted rate of $400/hr).
- Two 30-minute follow-up visits per year (extra follow-ups at a discounted rate of $400/hr)
- Medication prescribing
- Care coordination when indicated (communication with other providers, etc.)
Check-ins do not roll over
Premium Membership
$150/mo
What You Get:
